Infertility
Infertility may be defined as the failure to conceive after one year of continual unprotected sexual intercourse. In Hong Kong, approximately one out of every six couples suffers from infertility. It is advisable for women aged over 35, with irregular periods or history of gynaecological disease to see a fertility specialist if they fail to conceive after six months. We shall assess the couple together and find out the problem and provide the necessary investigations and treatment.

Investigations for Infertility
- Blood tests to check the hormones for ovulation and ovarian reserve; chromosomal analysis
- Ultrasound examination of the genital tract:
- ovarian reserve
- assessment of ovulation
- polycystic ovarian syndrome
- uterine factor for infertility: fibroids, endometrial polyps, congenital uterine abnormalities, endometrial thickness
- ovarian cysts
- Tubal assessment by hysterosalpingograhy or laparoscopy
- Sperm examination by computer assisted sperm analysis
Management of Infertility
The management depends on the underlying causes, age of the female partner and previous treatment response, including:
- Medication: usually used for induction of ovulation for women with ovulatory problem
- Surgery:
- tubal blockage or adhesion
- endometriosis especially with chocolate cysts
- ovarian drilling for polycystic ovarian syndrome
- uterine fibroids
- endometrial problems: polyps or adhesions
- Assisted Reproductive Technologies:
- artificial insemination/intrauterine insemination with or without ovulation induction (carefully prepared sperm from husband is injected into the uterus of the woman)
- In-vitro fertilization and embryo transfer (IVF or test-tube baby)
- Pre-implantation Genetic Diagnosis (PGD) / Screening (PGS)
